Chicago creates affordable health care for Hispanic seniors

Oak Street Health is a new network of primary health clinics for seniors in Chicago. Even though the network opened its clinics early this September they have experienced a large volume of patients.

With two locations in the city, Portage Park (4848 W. Irving Park Ave.) and Edgewater (1541 W. Devon Ave.), Oak Street Health offers adults enrolled in Medicare an excellent healthcare experience tailored to address their unique needs.

Oak Street Health was founded to offer older adults a full range of primary care services to help them improve their health and serves Medicare enrollees regardless of income level and at no additional cost beyond their normal Medicare plan.

“Our doctors take the time to listen to each patient, understand their individual circumstances and develop comprehensive care plans for each member,” said Mike Pykosz, Oak Street Health’s chief executive officer.

“We are passionate about bringing Chicagoans this proven holistic approach to medicine that can double the use of preventative medicine, lower emergency room visits by half and reduce some complications by over 90 percent,” said Griffin Myers, Oak Street Health’s chief medical officer.

Each Oak Street Health clinic is organized into dedicated care teams with one geriatrician, one nurse, one nurse practitioner and one care coordinator per team with up to three teams per center, plus two welcome coordinators, two member relations staff members and one office manager to serve a panel of 2,000 members. There is one doctor in each location that speaks fluent Spanish – Dr. Geraldine Luna and Dr. Tito Peña Guzman. The languages spoken at the clinic are English, Spanish, German and Polish.
